IX. RENT STABILIZATION. Chapter 13.76
This packet, to be filled out by the Head of the Household, is required to report any changes in income, assets, or household composition, and should be submitted within 14 days of occurrence.
Before starting this packet, please submit the Family Notice of Change(s) form and then check your email for a copy of the completed form for your records. To verify that you’ve completed the Family Notice of Change(s), you will be required upload that copy into this form.
Please be prepared to enter the email addresses of all adult household members when prompted below. Each household member will receive an email with a link to digitally sign the forms within this packet, as required. Once all signature requirements are fulfilled, you will receive an email with a copy of the completed form for your records.

This form, available as a PDF below, must be filled out and mailed, faxed, or dropped off in-person to Berkeley Housing Authority offices. It authorizes Section 8 Housing Assistance Payments (HAP) for rental property owned by multiple persons/entities to be paid to a designated party. This form is required for all properties owned by more than one party (married couples exempt) and must be notarized. All persons/entities on the title for the property (all legal owners) must sign this authorization form.

Mutual Agreement for Termination of Assisted Lease and Termination of Housing Assistance Payment Contract
This Mutual Agreement is to be initiated by the tenant, who will fill out and sign the form first. When filling out the form, the tenant must provide the owner/agent’s email address. The owner/agent will then be sent a link to also sign the Agreement. Once all signing requirements are fulfilled, the tenant will receive a confirmation email with a copy of the completed form.
The Mutual Agreement must be received by the Housing Authority at least ten (10) days before the original termination date. Please allow up to 20 days for processing.
